Calidad del sueño y somnolencia diurna en la epilepsia: una revisión sistemática y metaanálisis actualizado de 26 estudios que incluyeron 6,318 pacientes

Introducción: Las alteraciones del sueño son frecuentes en pacientes con epilepsia y puede afectar la calidad de vida. El objetivo del presente estudio fue sintetizar cuantitativamente la evidencia sobre la calidad del sueño y la somnolencia diurna comparado con controles sanos. Métodos: Se realizó una revisión sistemática y metaanálsis, se incluyeron estudios observacionales que comparan estas poblaciones utilizando Pittsburgh Sleep Quality Index (PSQI) y/o la Epworth Sleepiness Scale (ESS). Se calculó la diferencia de medias estandarizadas al ser escalas subjetivas en modelo de efectos aleatorios con ajuste Hartung-Knapp-Sidik-Jonkman. Se evaluó heterogeneidad y sesgo de publicación mediante GRADE. Resultados: Se incluyeron 26 estudios con un total de 6318 pacientes (2023 con epilepsia y 4296 controles sanos). El metaanálisis mostró peor calidad subjetiva del sueño (PSQI: SMD −0.54; IC 95%: −1.04 a −0.04; p = 0.04). Para la somnolencia diurna, se observó incremente pequeño pero significativo (ESS: SMD 0.26; IC 95%: 0.07 a 0.46; p = 0.01; I² = 77%). Conclusiones: Los pacientes con epilepsia presentan peor calidad de sueño y aumento de somnolencia diurna. Esto resalta la importancia de evaluar sistemáticamente el sueño como rutina en la consulta privada
